Sample Extraction Set

Get a free sample extraction set at info@genosense.com or call us at +431 2530253 190. Buccal Swaps

Each sample extraction set contains:

  • an order form
  • medical history forms
  • an easy-to-follow instruction sheet
  • three buccal swabs
  • the Return-to-sender-envelope

Please fill in the forms with great caution. Use capital letters.

To perform the requested analysis we need the correctly completed order form including the patient signature, the completed medical history form and both buccal swabs. All forms and swabs are bar-coded to insure confidentiality and privacy.

 

Forwarding the Report - a delicate matter

After the laboratory analysis and the medical interpretation are finished, a report will be generated and exclusively sent to you (the assigning physician) by mail or courier service.
